description Ocular coloboma can involve various structures, e.g., iris, lens, eyelid, zonules, retina-choroid, and optic disc. Here, we have discussed a rare co-occurrence of macular coloboma and optic disc coloboma (ODC) in the same patient. A 15-year-old boy presented with diminished best-corrected visual acuity measured in the right eye (OD) was 20/20 N6 and 20/60 N10 in the left eye (OS). On dilated fundoscopy, in OD, a large, deep hole with sharp borders on the temporal edge of the optic disc without any retinal tissue herniation was noted, while in OS, two large sharply demarcated excavations near the inferior arcade were evident. The larger one was measuring 5350 µm × 3928 µm. Detailed optical coherence tomography and fundus autofluorescence-based documentation were completed. This co-occurrence of double macular coloboma with contralateral ODC, in the history of ophthalmic literature, is considered extremely rare and has never been reported before to the best of our knowledge.
