    Diagnostic value of serum Cystatin C for contrast-induced nephropathy:A Meta-analysis by WANG Yu-qin, LUO Chang-qing, WANG Yu-mei, LI Zhen-qiong

    Published 2016-01-01
“…Objective To evaluate the diagnostic value of serum Cystatin(Cys) C for contrastinduced nephropathy(CIN) in the clinical practice by Meta-analysis and discuss its meaning. Methods The PubMed,EMbase,Ovidsp,The Chinese Biomedical Literature Database,CNKI,Wanfang,VIP and other databases were searched According to the requirements the retrieved literatures were screened step by step. The quality of the selected literatures was assessed by the instrument of quality assessment of diagnostic accuracy studies(QUADAS).The data of the selected literatures were extracted and analyzed using Meta-DiSc software to calculate their pooled sensitivity, specificity and other indices. The diagnostic features of Cys C for CIN were evaluated by drawing symmetric receiver operator characteristic(SROC) curve. Results According to the requirements there were 9 articles including 2165 patients. The Meta-analysis showed that the pooled sensitivity and specificity of serum Cys C to diagnose CIN were 92%and 89%,the pooled positive likelihood ratio and negative likelihood ratio were 7.33 and 0.12,the pooled diagnostic odds ratio was 71.14,the area under the SROC curve was 0.9539,and Q index was 0.8958,showing that serum Cys C had good diagnostic capabilities for contrast media in the early and that the extent of damage of eGFR could be assessed by testing serum Cys C level clinically. Conclusions The present study suggests that as compared with serum creatinine clinically, serum Cys C is more sensitive to reflect renal toxicity of contrast media in the early. So to some extent serum Cys C has the diagnostic value. But the reliability of the study needs to be proved by the clinical multi-center study with a lot of samples.…”

Meta-analysis of cinacalcet plus calcitriol for the treatment of secondary hyperparathyroidism in hemodialysis patients by Feng Hui-ling, Zhang Lian, Zhang Jin-ming, Wu Xiao-yan

    Published 2022-06-01
“…ObjectiveTo evaluate the efficacy and adverse reactions of cinacalcet plus calcitriol in the treatment of secondary hyperparathyroidism in hemodialysis patients.MethodsClinical trial of cinacalcet plus calcitriol in the treatment of secondary hyperparathyroidism in dialysis patients were retrieved from the databases of PubMed,Embase,Cochrane Library,China National Knowledge Internet,Wangfang and VIP.Two researchers independently screened the literatures and extracted the relevant data according to Cochrane Systematic Evaluator's Manual 5.1.0.The included randomized controlled trials were Meta-analyzed by RevMan5.3 software.ResultsA total of 30 articles were retrieved. The total sample size was 2497 people.Meta-analysis indicated that the effective rate of combined drug group significantly improved as compared with single drug group. And the effects of reducing the levels of serum phosphorus concentration, alkaline phosphatase, fibroblast growth factor 23 and parathyroid hormone and lowering parathyroid volume were more significant. The incidence of adverse reactions was lower in combined drug group than that in single cinacalcet group and there was no difference with single calcitriol group. The level of serum calcium was higher in combined drug group than single cinacalcet group and lower than that in single calcitriol group. The level of serum calcium-phosphorus product was higher in combined drug group than single cinacalcet group and lower than that in single calcitriol group.ConclusionBased upon current evidence, cinacalcet plus calcitriol in the treatment of secondary hyperparathyroidism offers a better clinical efficacy and a lower incidence of adverse reactions as compared with single medication in hemodialysis patients.…”
