    Changes and significance of urinary liver type fatty acid binding protein level in patients with AKI by QIAN Jian, XU Peng, ZHANG Jun

    “…Objective To investigate the changes and significance of urinary liver type fatty acid binding protein(L-FABP) level in patients with acute kidney injury(AKI).Methods Eighty-three cases of severe hospitalized patients from October 2013 to December 2015 in our hospital were selected,and divided into AKI group and non-AKI group according to whether there existed AKI on admission.The differences in biochemical indexes and urinary L-FABP levels between AKI group and non-AKI group were compared.The correlation between urinary L-FABP levels and biochemical indicators and ROC curve for urinary L-FABP level predicting AKI were analyzed.Results The serum urea nitrogen,serum creatinine,NAG and Cys C in patients with AKI of first examination after admission were higher than in non-AKI patients,and the difference was statistically significant(P<0.05).The glomerular filtration rate in patients with AKI was significantly lower than in non-AKI patients(P<0.05).The urinary L-FABP levels began to rise in the AKI patients on the 12 h after admission,and those in the non-AKI patients remained at a relatively stable level.The urinary L-FABP levels in patients with AKI were higher than those in non-AKI patients on 12,24 and 48 h after admission,and the difference was statistically significant(P<0.05).Urinary L-FABP levels in AKI patients on 12 h after admission were positively correlated with serum urea nitrogen,serum creatinine,NAG and Cys C of first examination after admission,and negatively correlated with glomerular filtration rate.ROC curve analysis revealed that the diagnostic threshold was 49.46,the sensitivity was 60.0%,the specificity was 95.83%,the Youden index was 0.558,and the area under the ROC curve was 80.89%.Qmclusions The urinary L-FABP level in patients with AKI was abnormally elevated,and it can be used as an important clinical index to predict the occurrence of AKI.…”
