Related factors and prognostic significance of predialysis blood pressure variability
Published 2018-01-01“…Objective To study the clinical variables that may plausibly influence predialysis systolic blood pressure variability(SBPV),and to investigate the correlations of predialysis SBPV with all-cause mortality in patients receiving prevalent maintenance hemodialysis(MHD).Methods A total of 50 patients were enrolled in the study.All the blood pressure values before each dialysis were recorded between March and May in 2011.The mean systolic pressure(SBP)was calculated,and SBPV was estimated with the coefficient of variability.During the three months,clinical data and related biochemical parameters were collected,and echocardiography was carried out to detect cardiac structure and function.Death events were recorded during the next five years.Results The predialysis SBPV was 8.5% ±2.1%.SBPV showed a positive correlation with the age,body mass index(BMI)and left atrial diameter(LAD)(P<0.05 for all).Meantime,SBPV showed a negative correlation with the serum creatinine(SCr),average DBP,parathyroid hormone(PTH),doses of calcium carbonate and activated vitamin D(P<0.05).Patients with diabetes mellitus(DM),coronary artery disease(CAD)or taking alfablocker had higher SBPV(P<0.05).SBPV was used as a variate for conducting the multiple linear regression analysis,after adjustment,the seven variables of age,DM,CAD,BMI,LAD,SCr and taking alfa-blocker maintained their associations with predialysis SBPV(P<0.05).The equation of R~2=0.630.During 5 years of follow-up,12 patients died(24.0%).The Kaplan-meier survival analysis showed that the predialysis SBPV elevation was associated with the mortality rate(P<0.01).Conclusions Advanced age,the history of DM and/or CAD,lower SCr,higher BMI and LAD,and taking alpha-blocker were the independent risk factors of increased predialysis SBPV.The predialysis SBPV increase is associated with all-cause mortality in patients given prevalent MHD.…”

Dynamic and efficient vehicular cloud management scheme with privacy protection
Published 2022-12-01“…The vehicular cloud (VC) formed by vehicles is used for localization processing and consumption of traffic sensing data to achieve timely intelligent traffic management.The vehicle cloud is highly dynamic, self-organizing and timely, in which the identity and location privacy of vehicle users need to be protected as this poses challenges to the vehicular cloud management.A dynamic and self-organizing vehicle cloud management scheme based on the asymmetric group key agreement protocol was designed, where the vehicle cloud is automatically formed through the self-organized group key agreement of vehicles.The group key was used to control the provision and access of vehicle cloud services, and the dynamic management of the vehicle cloud was implemented through group key update.The scheme used traceable one-time pseudonym technology to achieve anonymous authentication and conditional privacy protection of vehicle users, and the group key agreement stage only included one bilinear pair operation to achieve high efficiency.In addition, the key negotiation and update process used lightweight signatures, supporting batch verification, to achieve efficient message source authentication and integrity authentication.Then the security and efficiency of vehicle cloud communications in the self-organizing environment can be ensured.The dynamic key update mechanism of the key agreement protocol realized the dynamic joining or exiting of vehicles in the vehicle cloud, adapting to the dynamic characteristics of the vehicle cloud.Under the random oracle model and the difficult assumption of the inverse computational Diffie Hellman (ICDH) problem, it was proved that the asymmetric group key agreement scheme satisfied the selective-plaintext security.The security analysis shows that the scheme can protect the identity and location privacy of vehicle users, realize the legal tracking of malicious vehicles, and ensure the confidentiality, integrity and anti-counterfeiting of communications, as well as the forward security of vehicle cloud dynamic management.The performance comparison analysis shows that this scheme has certain advantages in communication and computing efficiency under the condition of the same function and security level.…”

Hemoglobin level and its related factors in patients undergoing peritoneal dialysis
Published 2021-01-01“…Results Mean Hb concentration was(106.08±20.42) g/L and the rate of target-attaining for hemoglobin 40.36%.The rate of targets for ferroprotein(200-500 μg/L),transferrin saturation(≥20%)were 76.78% and 38.95%.Compared with non target-hemoglobin group, target-hemoglobin group had significant statistical differences of dialytic duration, dose of recombinant human erythropoietin, subjective global assessment, serum phosphate, blood calcium, serum albumin, serum iron, ransferrin saturation, cholesterol level and residual renal function(P<0.05).Compared with high transport group, high average transport and low average transport groups had statistically different level of hemoglobin.Univariate logistic regression analysis revealed that duration of dialysis therapy >1 year, subjective global assessment, serum albumin<35 g/L,parathyroid hormone >300 ng/L,blood calcium >2.5 mmol/L,serum iron<10.6 μmol/L and transferrin saturation<20%.Multivariate Logistic regression analysis revealed that duration of dialysis therapy ≥5 years, serum albumin<35 g/L,intact parathyroid hormone >800 ng/L,serum iron<10.6 μmol/L and transferrin saturation<20% were independent risk factors of anemia in PD patients. …”
