    241 cases of idiopathic membranous nephropathy patients with clinical and pathological correlation analysis by XU Dan, LU Chen

    “…Objective To study the clinicopathological features and the factor of renal insufficiency with idiopathic membranous nephropathy(IMN).Methods The pathological data of 214 patients with biopsy-proven IMN who were treated in People’s Hospital of Xinjiang Uyghur Autonomous Region form January 2010 to March 2015 were analyzed retrospectively.The patients grouped according to the estimate glomerular filtration rate(eGFR) and 24 h urinary protein,analysising the clinical pathological characteristics and finding the risk factors for the decline of renal function.Results 214 IMN patients(171 males and 70 females,the male to female ratio was 2.4:1) involved,all of them has different levels of proteinuria,25 patients(10.4%) had proteinuria>10 g/24 h.There is a statistically significant on serum albumin(Alb),urine beta2-microglobulin(β2-MG),FSGS and renal tubular interstitial damage among the eGFR-group,smoking status and eGFR has significant among the 24 h proteinuria groups.Renal pathological changes were not related to sex.Pathological staging has positive correlation with age(r=0.137,P= 0.034),serum creatinine(r=0.208,P=0.001),24-hour urine protein(r=0.125,P=0.014),urine IgG(r=0.131,P=0.043),urine β2-MG(r=0.150,P=0.020) and renal tubular interstitial damage(r=0.238,P=0.035),negative correlation with Alb(r=-0.089,P=0.019) and eGFR(r=-3.331,P=0.000).Multiple regression analysis suggest that age,serum albumin,proteinuria level,urinary IgG and renal tubular interstitial damage are independent risk factors for renal function decline.95%CI were(1.066,1.158),(0.930,1.063),(1.904,2.545),(1.008,1.075) and(0.163,1.116).Conclusions The high stage of glomerular,renal tubular interstitial damage and renal function declined is obvious in IMN patients who was older,massive excret of proteinuria,IgG and urine β-MG.Which should be give positive treatment,smoking patients should be quit as soon as possible.…”
