    “…Introduction: Bone metastases occur in up to 70% of patients with advanced breast cancer, often necessitating bone-targeted therapies, such as denosumab, to reduce skeletal-related events.1 Denosumab, a monoclonal antibody against RANK ligand, inhibits osteoclast-mediated bone resorption, effectively stabilising skeletal integrity.2 However, this mechanism disrupts calcium homeostasis, particularly in patients with preexisting vitamin D deficiency, a condition prevalent in 30–50% of cancer populations.3 Severe hypocalcaemia, a life-threatening complication of denosumab, manifests in up to 18% of untreated vitamin D-deficient patients, escalating risks of cardiac arrhythmias, neuromuscular instability and prolonged hospitalisation.4 Despite international guidelines mandating pre-treatment vitamin D optimisation, clinical implementation remains inconsistent.5 This case report highlights the consequences of such oversight, demonstrating how protocol gaps can precipitate dual electrolyte crises (hypo- and hypercalcaemia) and advocating for institutional reforms to improve patient safety. …”
