Coflex棘突间植入与后路椎间融合固定治疗单节段腰椎退行性疾病的对比研究

“…【目的】 探讨单节段棘突间Coflex装置植入治疗腰椎退行性疾病的临床疗效【方法】 收集2008年6月至2009年12月符合入选和剔除标准病例92例,随机并结合病人意愿分为两组,21例采用单节段棘突间Coflex植入术 (Coflex组),71例采用单节段后路椎间Cage植骨融合术 (PLIF组)统计分析两组的住院日手术时间失血量组内及组间术前及末次随访的VAS腰痛评分VAS腿痛评分ODI功能评分手术节段和上邻节段ROM【结果】 Coflex组平均住院日手术时间手术失血量少于PLIF组 (P < 0.01);末次随访两组VAS腰痛评分VAS腿痛评分ODI功能评分均小于术前 (P < 0.01),术前末次随访组间差异无统计学意义 (P > 0.05);末次随访Coflex组手术节段ROM与术前差异无统计学意义 (P = 0.70),末次随访PLIF组达到骨性融合,两组术前手术节段ROM差异无统计学意义 (P = 0.50),Coflex组末次随访ROM明显大于PLIF组 (P < 0.01);末次随访Coflex组上邻节段ROM与术前差异无统计学意义 (P = 0.63),末次随访PLIF组ROM明显大于术前 (P = 0.02),术前两组上邻节段ROM差异无统计学意义 (P = 0.61),末次随访PLIF组ROM明显大于Coflex组 (P = 0.02)【结论】 单节段棘突间Coflex 装置治疗腰椎退行性疾病在改善临床症状方面与后路椎间融合固定术无明显差异,但平均住院日手术时间和术中失血量较少;棘突间Coflex 装置能保留可控的手术节段椎间活动度,并有效减少上邻节段椎间活动度…”

URSA 患者外周血免疫细胞特征及其意义的临床研究

“…【目的】 探讨不明原因复发性流产(URSA)患者处于难免流产和流产后非孕状态等不同临床情况时外周血免疫细胞特征及其临床意义65377; 【方法】 回顾性分析2008-2010年于我院诊疗的自然流产患者184例,就诊时为早期妊娠难免流产的URSA患者55例(URSA难免流产组),同期早期妊娠难免流产的偶发流产患者(sporadic abortion, SA) 23例,作为对照(SA难免流产组)65377;此外,就诊时为未孕状态,距最后一次流产3个月以上URSA患者62例(URSA非孕组),同期未孕状态,距流产3个月以上SA患者44例作为对照(SA非孕组)65377;记录患者一般情况,包括年龄65380;月经周期65377;使用流式细胞术检测外周血免疫细胞,包括T淋巴细胞亚群:T细胞比例65380;Th细胞比例65380;Ts细胞比例65380;Th/Ts;其他免疫细胞:白细胞数量65380;淋巴细胞总数量(LYM细胞)65380;B细胞比例65380;自然杀伤细胞(NK细胞)比例65377;【结果】 难免流产状态:URSA患者与SA患者Th/Ts均异常升高,前者为2.77 ± 5.16,后者为3.67 ± 6.12,URSA患者与SA患者比较,前者NK细胞比例明显升高,为(15.57 ± 6.45)%,后者仅为(11.38 ± 3.66)%,差异有统计学意义(P < 0.05)65377;非孕状态:URSA患者与SA患者比较,URSA患者WBC数量及NK细胞比例高于SA组,前者为(6.27 ± 1.09) × 10965380;(18.26 ± 7.88)%,后者为(5.59 ± 1.16) × 10965380;(11.0 ± 6.8)%,差异有统计学意义(P < 0.05)65377; 【结论】 在发生早期妊娠难免流产时,URSA患者与SA患者均存在一定的免疫紊乱,共同的表现是特异性免疫方面均有Th/Ts异常升高,不同的表现是,URSA患者在非特异性免疫方面存在NK细胞比例增高65377;在非孕状态,SA患者免疫紊乱得到调整回复正常,URSA患者仍然存在一定的免疫紊乱,主要表现在非特异性免疫方面,以WBC数量和NK细胞比例增高为主65377;临床工作中应注意监测URSA患者的免疫状态65377;…”
