Summary:Purpose: To evaluate the differences in intraocular lens (IOL) decentration, tilt, and lenticular astigmatism between Yamane’s IOL fixations combined with pars plana vitrectomy (PPV) or anterior vitrectomy (AV), as well as compare those results with standard phacoemulsification surgery at the sixth month. Methods: This prospective longitudinal comparative study included 20 eyes treated with Yamane’s IOL fixation combined with AV (AV group), 20 eyes treated with Yamane’s IOL fixation combined with PPV (PPV group), and 20 eyes that underwent standard phacoemulsification with the implantation of a foldable IOL in the capsular bag as controls (Phaco group). IOL decentration, tilt, and lenticular astigmatism values were compared between the groups. Results: The horizontal tilt angle was statistically significantly lower in the Phaco group compared to the PPV and AV groups, while no significant differences existed between the PPV and AV groups. The mean values of horizontal decentration, vertical decentration, and vertical tilt 6 months postoperatively showed no statistically significant differences between the three groups (P > 0.05 for all), while all parameters were lower in the Phaco group. Moreover, no statistically significant differences existed in accordance with the mean lenticular astigmatism 6 months postoperatively between the three groups (P = 0.969). Conclusion: AV or PPV combined with Yamane’s technique exhibited similar IOL decentration, tilt, and lenticular astigmatism at 6 months.
