| Summary: | Objective To investigate risk factors of contrast-induced nephropathy(CIN) in patients with coronary heart disease undergoing coronary angiography,and provide the basis for preventing and improving the prognosis.Methods 1115 patients undergoing coronary angiography were enrolled in this study,and divided into CIN group and non-CIN group according to changes in serum creatinine levels within 24 h-72 h after percutaneous coronary intervention.Both groups were given loversol and underwent hydra tioa Data on age,gender,smoking,diabetes,contrast agent dose,chronic kidney disease,hypertension,hyperlipoidemia,heart failure etc,and risk factors of CIN were analyzed by Logistic regression analysis.Results Eighty-three cases of CIN were found in 1115 patients with coronary heart disease who underwent coronary arteriography with the incidence of CIN being7.44%.Multivariate Logistic regression analysis showed that advanced age(OR = 1.005,95%CI:1.005- 1.048,P = 0.002),chronic kidney disease(OR = 5.977,95%CI:4.652- 6.054,P =0.000),diabetes(OR=2.077,95%CI:1.214-3.555,P = 0.008),and contrast agent dose(OR =2.020,95%CI:1.240-3.293,P = 0.005) were the independent risk factors for CIN.For gender,OR = 0.858,95%CI:0.599-1.229,P = 0.403;smoking,OR = 0.858,95%CI:0.558-1.053,P= 0.101;hypertension,OR = 1.141,95%CI:0.825-1.577,P = 0.426;hyperlipoidemia,OR =1.066,95%CI:0.884-1.287,P = 0.502;and heart failure,OR=0.789,95%CI:0.494-1.261,P = 0.322.Conclusions Advanced age,chronic kidney disease,diabetes and contrast agent dose were the independent risk factors for CIN in patients with coronary heart disease after percutaneous coronary interventioa Especially,rigorous preoperative preparation and hydration were important prevention strategies for CIN. |
