description Introduction. One of the main factors involved in bone mineral and musculoskeletal disorders in patients with chronic kidney disease (CKD) is fibroblast growth factor-23 (FGF-23). Fibroblast growth factor 23 (FGF-23) levels are elevated in hemodialysis patients. Despite FGF-23 associated left ventricular hypertrophy, there are no prior studies assessing whether FGF-23 level is associated with skeletal muscle strength in hemodialysis patient. This study aimed to determine the correlation between hand-grip strength (HGS) and FGF-23 levels in patients undergoing twice weekly hemodialysis patients. Methods. A cross-sectional study was conducted on patients with twice weekly hemodialysis at the Hemodialysis Unit at Dr Cipto Mangunkusumo Hospital, Jakarta. We measured intact fibroblast growth factor-23 and hand grip strength that measured by Jamar hydraulic hand dynamometer. Subject selection was done by consecutive sampling until the required number of subjects was fulfilled. Results. A total of 74 subjects were included in this study which serum FGF23 levels were significantly higher in males (median 3,276 pg/ml [range 41-6,613 pg/ml]) than in females (median 1,585 pg/ml [range 21-4,820 pg/ml]). The HGS value of male patients was 25 kg (range 11-48 kg), while in female patients was 20 kg (range 8-26 kg). After adjusting for modified charlson comorbidity index (mCCI) and body mass index, there was no correlation between FGF-23 and HGS in male subject (r = -0.053, p = 0.7), but the correlation FGF-23 and HGS in the female subject was significant (r = -0.4, p = 0.02). Conclusion. In twice weekly hemodialysis patients, FGF-23 has a significant correlation with HGS in women with body mass index >18,5 kg/m2 and this correlation was not found in the male subjects.
