Summary:This research-action multicentrique has shown the changes bring by the implementation of patient education in hospital departments. In this article the modifications of the representations and the roles of the professionnels are discribed. Semi-directive interviews were realized, completed by statements of observation. To highlight the changes, data were collected in two phases over 18 months, added focus groups about the main emergent themes with the professional of each department. The first part handles the major principles of the ETP. In a second part, we describe the search-action driven with actors of care and with sick having for object the impact of the therapeutic education of the patient in diverse services. We handle then a part of the results of the research in classifying the speeches of the nursing around the ETP in three categories : the representations at the institutional level, at the level of the service and at the level of the individuals. Patient education obliges the caregivers to question their practices and raises contradictions in their representations and in their vision of their role. The research’s shown that they have to deal with tensions under severals angles, knowing that the concepts used in education are often against the health system paradigms and the role expected of them. Finally, an essential dimension of the transformations lies in the caregivers’acceptation of changing their role in education by their own.
