description With the industrial activities raising, the technology expanding and the use of machinery increasing, the accidents have boosted. All industrial operations and activities areseeking to fulfill requirements, the neglect of which can lead to unintended consequences. By identifying and assessing the risk, it is possible to identify accident-prone and critical points and avert the accidents and control them. Identifying and assessing the risk is an organized and systematic way in ranking, prioritizing and reducingthe risk. Today, risk assessment is highly important to control hazardand to improve health and safety of employees. Deciding about precautionary measures and reducing the risk to an acceptable level necessitate the assessment of the health risk caused byoccupational exposure in particular. The aim of this study is to evaluate the occupational health hazard with a Comprehensive Occupational Health Risk Assessment (COHRA) adopting data envelopment analysis technique. The current descriptive-analytical research has been carried out inan operational occupation in one of the Iran's petrochemical plants.The elementsExposure Rate (ER) and Hazard Rate (HR) are the input and "Risk Degree" is the output of the mentioned model. Judging by the results, it is expected thattheexisting risks level can be significantly reduced if precautionary measures are provided. The most important controlling solutions are to provide training, raise awareness, continuously monitor the work and improve the occupational safety and health management system.
