Summary:Intramuscular scapular abscesses are rare and challenging to diagnose due to their deep location and ability to mimic common shoulder pathologies. This case report presents a 55-year-old female who developed an intramuscular scapular abscess following multiple traumatic events, including a closed reduction of a dislocated shoulder. Despite initial conservative treatment and steroid injection, she developed pain, swelling and pus formation. Diagnostic ultrasound and microbiological analysis identified methicillin-resistant Staphylococcus aureus. Surgical drainage targeted antibiotics and led to significant recovery with some residual stiffness. This case highlights the importance of considering scapular abscesses in patients with shoulder pain and trauma, emphasising a multidisciplinary approach for optimal recovery.
